Headings and image descriptions
The outline search engines and screen readers both read.
One h1, in order
Exactly one h1 per page, naming what the page is about. Then h2, then h3, never skipping a level.
Heading level is about meaning, not size. Pick the level that fits the structure and use CSS to make it look right — choosing h3 because it was the right font size is what produces a broken outline.
alt="" is not the same as no alt
An empty alt is a valid, correct marker for a decorative image: it tells a screen reader to skip it. A MISSING alt attribute leaves the screen reader to read out the filename.
We only report images with no attribute at all. Describe what the image shows and why it is there — not "image of".
